No Annual Fee Credit Cards

I'm talking about the Sears credit card offers they make at the register. They denied me due to unfavorable credit history-- which I don't have, and recent credit applications-- which I haven't made. I checked my Equifax report and everything is normal-- the only reason I can think of is the opt-out but I can't confirm it.

If you fill out and credit card application and the status says "need to be reviewed more"

Then you check it after a few day and it says the same thing
Doesn't that mean your more than likely going to be approved, Because if they were going to deny me, they would have already right?

I filled out a capital one credit card app and it says instant approval but at the end it told me a i would get a decision in 7-10 days does that mean it was denied????
